I don't think Shang Tsung in MK1 is the Shang Tsung from MK11 Aftermath. He has none of the attire of Time Lord Shang Tsung, not even the Crown which gave him his powers (plus they changed his design). IMO he is simply this new era's Shang Tsung doing usual Shang Tsung scheming despite Liu's best efforts. And it's the same with the disease.
Shang Tsung was truly awesome in MK 11 Aftermath, and other medias like the '95 movie. Overall, however, I think Shao Kahn is the better villain.
